Insurance Information

We accept all PPO dental plans as an out-of-network provider.

While this may mean slightly higher out-of-pocket costs, it allows us to prioritize long-term personalized care – not restricted by insurance limitations.

  • We file claims on your behalf to help maximize your benefits.
  • Full payment is required at the time of service.
  • Your insurance will reimburse you directly for any covered amount.
  • Need a treatment estimate? We’re happy to submit a pre-treatment plan to your insurer – please allow 2 weeks for a response.

** Please note: Your insurance contract is between you and your provider. While we advocate for you, final responsibility for payment and follow-up with insurance lies with the patient.

We are happy to assist you in obtaining maximum dental benefits by preparing and submitting your claims.

Please note that we accept all insurance PPO plans as an out-of-network provider, which means your out-of-pocket expense may be higher compared to in-network dentists, but this allows us to spend the necessary time on a prevention-focused approach and recommend long-lasting dentistry that prioritizes your health over insurance limitations.

We require payment in full at the time of service. Your insurance provider will send you a reimbursement check for the portion of your care they cover. If the insurance carrier disputes payments, we will do our best to advocate for you. Still, we cannot be responsible for collecting your insurance benefits or negotiating a settlement of a disputed claim after a reasonable time. Your insurance policy is an agreement between you and the insurance company, and ultimately, you are directly responsible to our office for all charges due.

At your request, we will submit a copy of your treatment plan to your insurance carrier so that you can receive an estimate of benefits before starting treatment. Most insurance companies take about two weeks to provide an estimate, so please allow sufficient time for your request. It is important to note that this predetermination of benefits is not a guarantee of payment by your insurance carrier, and ultimately, the total cost of your treatment is your responsibility.

Office Financial Policies

To best serve you and all of our patients, we have implemented the following policies:

Payment Due at Time of Service

Payment is required in full at each visit, even if you have insurance.
If you have insurance, your insurance provider will send you a reimbursement check for the portion of your care they cover.

Insurance

We accept all PPO insurance plans as an out-of-network provider, which means your out-of-pocket expense may be higher compared to an in-network dentist. We do this to spend the necessary time on a prevention-focused approach and recommend long-lasting dentistry that prioritizes your health over insurance limitations.
