New Patient Information
Our practice is committed to providing you and your family with safe, gentle, high-quality dental care. We understand that you or your child may feel anxious about visiting the dentist. We are sensitive to your needs, and it is our goal to make you feel comfortable visiting our practice while providing you with the best care possible.

Your First Visit to Seattle Dental Associates
Your first visit is all about you – your comfort, your happiness, and your health.
During your first visit, we will:
- Review your dental and medical history forms
- Take X-rays of your teeth and mouth
- Check your teeth and gums for any cavities or early signs of tooth decay or gingivitis
Dr. Baker will meet with you to discuss your X-rays, help you create your own individual treatment plan, and answer any questions that you may have. If it is important for you to have a cleaning at your initial visit, please let one of our team members know and we will do our very best to get you taken care of.

Financial Information
Your smile is yours forever, and we want to make it as easy as possible for you to receive the best dental care whenever you need it.
The cost of your treatment may vary depending on your individual needs and treatment plan. We will discuss the cost of your treatment and each of your available payment plan options with you before you begin, so you can make the best choice for you, your smile, and your family.
We provide you with options for consolidating treatment into as few treatments as possible, in addition to financial arrangements you are comfortable with.
Ask us how we can help or check out our in-house dental membership plan to see how we prioritize your dental care while keeping it affordable.

We offer individual dental plans through memberships. It’s like joining a club like Costco to get discounts. These dental membership plans give you benefits that make dental care easier to afford. Overall, dental membership plans provide a simpler, more flexible, and often more cost-effective alternative to traditional dental insurance, making dental care more accessible and manageable for many patients.

We are pleased to offer you several financing options and our financial coordinator will work directly with you to make sure we find a solution that best fits your dental health needs as well as your budget. Because many dental insurance plans don’t fully cover the cost of treatment, we accept the following payment options: cash, check, credit cards, and cherry financing.

We accept all PPO dental plans as an out-of-network provider. While this may mean slightly higher out-of-pocket costs, it allows us to prioritize long-term personalized care – not restricted by insurance limitations. As a courtesy to you, our administrative team will submit claims to your insurance company on your behalf.
Nine Reasons to Choose an Out-of-Network Dentist
Our In-House Dental Membership Plan is designed to give you top-tier dental services at affordable rates. Say goodbye to insurance limitations and hello to personalized care that puts your needs first. Want to know how our plan can benefit you?
Accepted Dental Insurance Plans
Our goal is to make sure your entire experience with us is as comfortable as possible, and we understand that financing your dental care is part of that.
At Seattle Dental Associates, we gladly accept all PPO dental plans as an out-of-network provider. This gives us the freedom to focus on prevention-first, personalized care—not limited by insurance restrictions.

Out-of-Network Benefits
While your out-of-pocket costs may be slightly higher than in-network options, many patients find the quality of care and long-term value well worth it.
We Handle the Paperwork
Our experienced team will file your insurance claims on your behalf to help you get reimbursed quickly and maximize your benefits.
Payment Expectations
We require full payment at the time of service. Your insurance provider will then reimburse you directly for any covered portion.
Need a Treatment Estimate?
Upon request, we’ll submit a pre-treatment estimate to your insurance company. Most respond within two weeks, though this is not a guarantee of payment.
What Our Patients Are Saying
Read what our clients have said about what left them smiling whenever they leave our office!
Dr. Baker is attentive and discusses treatment plans, giving recommendations when needed without being pushy. Other dental staff are caring and constantly check if everything feels comfortable for the patient.
I came in for a cleaning and another procedure and learned the hygienist scheduled to clean my teeth had called in sick, which meant I'd have to return another day for the cleaning. Wasn't thrilled at that prospect since I travel a long distance. Amazingly, Dr. Phan was available so she cleaned my teeth, saving me an extra trip! I was so grateful at their flexibility.
The women who cleaned my teeth was amazing. I believe her name was Chelsea. She was so nice and we had a great time. She always made sure I was comfortable and if I needed anything. I’ve been coming here for about 9 years and same with my family. Great place to go!
First off they are great dentists and dental hygienists. Their receptionists are wonderful too. What really sets them apart is that they are so personable. If you are someone who has a fear of dentists, this is definitely the place to go. They are state of the art in their procedures and their equipment. I am very pleased with their service.
Dr. Baker and team are amazing. I have been going here for 20 years and they take great care of me.
